Please complete the following form if you are applying to:

  • the Bachelor of Fine Arts (BFA) program
  • the Bachelor of Arts program with a major or minor in Fine Arts (BA major/minor, Fine Arts)

You must submit this form as well as your portfolio no later than Feb. 15.


Before you begin to fill in this form, make sure you have:

  • explored the different Fine Arts degree options and decided to which you are applying
  • read and chosen which personal response questions to answer; it is recommended you compose the answers offline and copy and paste them into the form
